Official transcripts for students can be requested online through Parchment. Information is available via the Office of the University Registrar. All students and alumni who were enrolled in a School of Medicine degree program (August 2004-present) must add the Washington University in St. Louis School of Medicine as an additional school attended in Parchment in order to request an official transcript. The ‘Allied Health’ option should be selected. The office above should also be contacted for copies of diplomas and verification requests.
Current students, former students, and alumni of PACS (1914-present) can request copies of items contained in their educational record, such as practicum records and exam scores, by submitting a request to the PACS Office at pacs@wustl.edu.
